Notify your doctor of any allergies and sensitivity to LATEX. Notify your doctor if you or a family member have a history of high fever or convulsions with anesthesia (or history of Malignant Hyperthermia). Notify your doctor if you use any type of street or recreational drugs. Make arrangements for a responsible adult to drive and accompany you the day of surgery. Make arrangements for a responsible adult to stay with you when you get home the day of surgery. ALL piercings must be removed for your safety. If you are unable to remove them, have them professionally removed. If not, your surgery may be canceled. Notify your doctor if you are ill or have a fever prior to surgery. Notify your doctor if you need crutch walking training and get a prescription for physical therapy. Get this training BEFORE THE DAY OF SURGERY! If you are a smoker and are having general anesthesia, you will benefit by not smoking as long as possible before the day of surgery. If you have an abnormal EKG, a significant cardiac history, uncontrolled or newly diagnosed high blood pressure, uncontrolled diabetes, or significant pulmonary disease, you will be required to see your internist or cardiologist before having surgery. Have all required lab tests or other testing done AT LEAST 2 DAYS prior to surgery. Required pregnancy tests will be done at the center the day of surgery. This will require a urine sample upon admission. DO NOT TAKE any of the following for at least ONE WEEK prior to surgery:
