Bring your current insurance card. Bring co-pay or required deductible as determined by our insurance office. If you have not discussed this with our office, please call prior to surgery at Extension 323. BRING ALL MEDICATION BOTTLES, INSULIN AND INHALERS. Bring crutches, if applicable. Bring a NEW scrotal supporter/jock strap if you are having scrotal or inguinal surgery (includes hernia and vasectomy patients). If you are diabetic, please notify nursing staff of your morning glucometer reading the day of surgery. Do not wear jewelry of any kind (remove all piercings). Do not wear contact lenses, wear your glasses. Do not wear removable hair pieces, wigs, makeup, or nail polish. Wear your hearing aid. Wear COMFORTABLE clothing (loose fitting to cover large bandages, bulky dressings, casts, or splints). Fill prescriptions and take antibiotics before surgery as prescribed by your physician. DO NOT EAT OR DRINK ANYTHING THE DAY OF SURGERY (after midnight the night before, unless instructed otherwise by your physician, Anesthesiologist or a Calumet Surgery Center nurse). Take ONLY HEART, BLOOD PRESSURE, ASTHMA/COPD & ANTI-SEIZURE medication the morning of surgery with a very SMALL sip of water. Take other preparations as instructed by your physician. TRANSPORTATION ON THE DAY OF SURGERY.
**EYE PATIENTS ONLY** Wear short sleeve button down shirt/blouse the day of surgery. Bring eye kit given to you at the eye doctor's office. Fill eye drop prescriptions and bring eye drops the day of surgery.
